Before the surgery, the doctor will ask the patient to stop the intake of medicines that make it hard for the blood to clot, such as aspirin, ibuprofen, warfarin, and similar other medicines. The doctors will instruct to strictly stop the intake of any kind of food or drink a night before the surgery. Patients should keep in mind to keep at least 6 to 12 hours of gap between the last meal and the surgery. If the patient has to take any medicine, then it must be consumed only with a sip of water. 

On the day of the surgery, in the morning the nurses will monitor the patient's blood pressure, sugar level, and other checks up necessary like a few blood tests. Then, a sedative will be given to the patient to clear his stomach completely before the surgery. When the patient is ready for the surgery, he will be given general anesthesia so that he remains unconscious and unaware during the surgery.

There are four types of bariatric surgery, and a patient is suggested one depending on his overall health, BMI and other weight-related health issues. Following are the common gastric surgeries performed - 

1. Roux-en-Y: It’s the most common method of gastric bypass surgery which is not typically reversible. The surgeon cuts across the top of the patient’s stomach and seals it off from the rest of the stomach. After the surgery, the pouch is reduced to the size of a walnut, which is capable to hold only about an ounce of food. Then, the doctor operates on the small intestine, which is cut and sewed a part of it directly onto the pouch. After the surgery, the food enters directly into the middle portion of the small intestine.

2. Biliopancreatic diversion with duodenal switch: This type of gastric bypass is a complex procedure where about 80% of the stomach is removed. The parts which only remains are the valve that releases food to the small intestine (the pyloric valve) and a portion of the small intestine that generally connects to the stomach (duodenum). Maximum part of the small intestine is bypassed in this surgery and only the end portion of the intestine is connected to the duodenum near the stomach (duodenal switch and biliopancreatic diversion). This surgery is usually recommended to people whose BMI is greater than 50.

3. Laparoscopic adjustable gastric banding (LAGB): an inflatable band around the uppermost part of the stomach is positioned in this gastric bypass surgery. Acting like a belt, the band compresses the stomach when it is inflated. The surgery separates the stomach into two parts, where one part acts like a very small upper pouch that works with the rest of the stomach through a medium created by the band. The band is adjustable to restrict more or less intake of food.

Sleeve gastrectomy: Also called a vertical sleeve gastrectomy is a new invention in the field of gastric bypass surgery and in some cases it is the first part of the surgical procedure for a biliopancreatic diversion bypass surgery. In this surgery, the actual structure of the stomach is changed into a tube-like structure, which helps in restricting the number of calories the body would absorb.

After the surgery is over, the patient is brought to a recovery room under the observation of the medical staff and nurses where the patient is made to stay until the anesthesia wears off completely. Post that, the patient is transferred to a normal room for about 1 to 4 days depending on their recovery. It should be noted that post the surgery, the patient is not allowed to eat for one to two days to let the stomach and digestive system heal completely. After one to two days, the patient will be given a specific diet which has to follow strictly for about 12 weeks. The diet initially starts with liquids only, then progresses to ground-up or soft foods and then lastly to regular foods. The patient will have restrictions on what and how much to eat or drink.  

The patient will be made to wear special stockings on legs to prevent the formation of blood clots. The patient will also be asked to take specific medicines to prevent blood clots.
