If you have been recommended by your doctor to undergo colectomy, the following steps should be followed to ensure you are well prepared for your surgery. 

Medications – Consult your doctor to know which medications to stop before the surgery. It is important to discuss this as certain medicines may increase the risk of complications during surgery.

Fasting – You may be asked to fast for several hours to a day before the surgery

Clearing your bowels – You may be given a laxative solution a day or two before your surgery. The solution will help clear the bowels by causing diarrhea. Your surgeon may also recommend you to have enemas.

Alcohol & Smoking: You would be required to stop alcohol as well as smoking 1-2 weeks prior to the procedure as well as for several weeks after the procedure or else it could drastically reduce the healing process and could cause complications during the procedure.

Preparing for colectomy isn't always possible. For instance, if you need an emergency colectomy due to bowel obstruction or bowel perforation, there may not be any time to prepare.

In the procedure of colon surgery, a GI surgeon may remove parts of the colon, lymph nodes, etc from your intestines, cut away tumors which are obstructing the bowel, join the different parts to each together or create an opening outside the body to ease your problem. The type of surgery conducted would depend upon your colon's condition and should be discussed with your doctor extensively.

At first, you will be taken to the preparatory room where your blood pressure and other medications will be monitored. You will be given anesthesia to help you relax your mind and body and to put you in a sleep-like state.

There are different types of colectomies, some of them are given as under: 

Open colectomy – In this type of surgery, your surgeon creates a big incision in your stomach to access the colon. He then frees it from the surrounding tissues and cuts it entirely or in part, depending on your condition.

Laparoscopic colectomy – In this type of surgery, the doctor will make several small incisions in your abdomen. He will then pass a laparoscope ( a thin tube with a camera at the end) through one of the incisions. The surgeon will watch the insides of your colon on a video screen and using special tools free it up from the surrounding tissue and makes the necessary repairs. Once repairs are made to the colon, the surgeon will reinsert the colon through the incision.

Once the colon has been repaired or removed, your surgeon will follow any of the following procedures to allow your waste to expel out. Options may include:

Joining the remaining portions of your colon together - The surgeon may stitch the remaining portions of your colon together. Stool then leaves your body as before.

Connecting your intestine to an opening created in your abdomen- The surgeon may attach your colon to an opening created in your abdomen. This procedure is also known as colostomy. The waste then leaves your body through this opening.

You may wear a bag on the outside of the stoma to collect stool. This can be permanent or temporary.

Connecting the small intestine to anus – The doctor may even directly connect the small intestine to the anus. Depending on your condition, the surgeon will choose the suitable option.

Your surgeon will discuss your options with you before your operation.

After the procedure is completed, the following steps will be performed by a medical professional or nursing staff:

Post-surgery you'll be taken to a recovery room to be monitored as the anesthesia wears off. You will be kept in the hospital room until you achieve a full recovery and bowel control. This may take from 2 to several weeks.

If you have had a colostomy, the nurse will show you how to take care of the stoma outside your body and change the bag.

You won’t be able to eat solid foods for some time so a vein will be connected to your body. This vein will provide liquid nutrition to your body until you regain your bowel control.

Recovery at home:

Once you leave the hospital, it may take a few weeks before you regain your complete strength. Rest as much as possible during these few weeks.

• Eat semi-liquid foods to avoid indigestion

• Avoid doing any vigorous exercises

• Avoid eating outside for a few months

• Avoid smoking and drinking alcohol

• Consult your doctor if you feel any sort of severe pain while recovering.

Colectomy is a safe procedure if performed by qualified experts. However, certain risks are associated with this procedure. The following is a set of such complications:

• Infection

• Bleeding

• Damage to adjacent structures

• Heart attack or stroke

• Blood clots

• Difficulty in breathing

• Pneumonia

The risks that are specific to lower bowel colectomy include:

• Bleeding inside the abdomen

• An incisional hernia, which occurs when tissue comes out through the surgical cut

• Damage to the bladder or other nearby organs

• Scar tissue

• Opening of a surgical wound

• Skin irritation near the surgical site

There are also risks associated with general anesthesia. These include reactions to medications and breathing difficulties.
